Self Monitoring Blood Pressure Program

Has a doctor told you that you have high blood pressure or hypertension?
If you answered yes, do you need support in managing your high blood pressure at home? We can help!

Self-Monitoring Blood Pressure Program (SMBP). High blood pressure is known as the “silent killer” because it usually has no symptoms and cannot be detected without being measured. When left unmanaged, high blood pressure is a dangerous condition that increases the risk for heart disease and stroke.

Visit the Mapu Maia clinic’s hypertension clinic days (2nd Wednesday of each month) to receive a blood pressure screening and learn how to keep yourself in a healthy range.

If you need support, you can participate in a self-monitoring blood pressure program to track at home and receive follow-up care.  Email diana@utopiawa.org for more information and to make an appointment.
